download the form here but to make sure we award Housing and/or Council Tax Benefit from as early date as possible, you must register your intention to claim. You can do it online by selecting the option 'I want an application form' or you can contact us.

Change in Circumstances
Download this form to report changes in your circumstances other than a change of address, such as a change of income.
Change of Address
Download this form if you want to claim Council Tax Benefit only.
