Chorley Council on-line Benefit Claim Form

You can now complete and submit a claim form for Housing and/or Council Tax Benefit on-line. But, before you start, please read these notes - you may want to print them out to refer to as you complete the form.

The on-line claim form asks a lot of questions.  If you want to save what you have done so far and then go back to it, ensure you have completed a page and then click 'save current information'. (It may be necessary to use the 'back' option to return to the last fully completed page and then 'save current information'). Make sure you note down the reference number and password – you will need these when you go back to complete the form via the 'Edit claim form' option. Please remember that any delay in completing the form and submitting it may lead to a loss of benefit.

In addition to these notes, there are Help Notes contained in the claim form when you start completing it. You can simply click on 'Help' at any time. These should also be read before completing the claim form and can be printed out if you wish.

  1. If you have previously claimed benefit from Chorley Council, please have your previous benefit reference number (starting 100) to hand. If you cannot find it/can't remember it, please contact us on 01257 515410 before continuing.
  2. If you want to claim Council Tax Benefit, you will need your Council Tax account number (starting with a 6). If you do not know it or have just moved in and don't have one yet, please leave this part blank.
  3. Please switch on your caps lock so the form is completed in CAPITALS.
PART 1 - About you and your partner

Question – 'If you have moved from this address have you told the council you claimed from?'
You will still need to answer this question even if you have not claimed before. In this case answer 'YES'.
if you have moved from one address in Chorley to another, please answer 'YES'.

PART 2 – About children (continued)

We DO need to know about all children you receive child benefit for. However, we do NOT need information about Children's savings – so answer NO to the question about savings for CHILDREN.

PART 10 –About where you live (continued)

Even if you answer NO to the question 'Do you have a main home somewhere else also', you must still answer the question ' Do you pay rent for this home' – in this case you should answer 'NO'.

Question – How long is the tenancy for?
Answer – If your tenancy is on-going and has no end date, enter a date of 31.12.2099 in the 'to' date.

PART 12 – About bank accounts, savings, investments and property

We do NOT need to know about the savings for CHILDREN

PART 13 – How you will be paid and the choices you have

If you want payments to be sent to your landlord, you do NOT need to complete the authorisation sheet at the end of the form.
Please note that if your claim is processed under the Local Housing Allowance rules, payments cannot, normally be sent to your landlord.

Enter the date where shown.

PART 18 – What to do next

In ALL cases, please send evidence in support of your claim to The Benefit Section at Chorley Council.
